First off let me say again sorry for not replying sooner. Next let me say I love the username you picked. Having the spine problems I have and after having 7 spine surgeries and several others.... I live my life for today so that is why I love the name.
I will try to help you as much as I can but I have to say at this point have I have little problems with my T-spine other then C7-T1 and T12-L1 where I have bugging disc's and some bone spurring (Osteophytes).
What kind of advice are you looking for? Would you like information about each of these conditions?
What caused these problems for you (an injury) or did they just happen? What levels of the T-spine do you have each of these?
I am going to reply to all of your posts in this reply if that is OK? I am trying to put a picture in my head of everything you have going on.
How many ESI's have you had and of which levels? Most often if they didn't work after a series of 3 then chances they will not help with the next. Don't get me wrong they have helped others but from my experience they did nothing. The other thing is most often if they help with pain it will only be temporary.
I am a bit confused if your doctor does the lumbar injections and they help why would he want to do surgery to free the L4-5 nerve? Usually if the injections helps they will wait to do surgery. What kind of surgery does your doctor want to do? Is a disc bulge pinching the nerves or is it the bone spurs (Osteophytes)? Should your doctor want to do a micro-D or a Laminectomy you may get a some relief of your back pain but don't count of a lot or for the back pain to be totally gone. The surgery alone may cause another type of back pain and the risk of scar tissue build up after surgery may also give you back pain or even more back pain. For some scar tissue is a big problem. I guess I was lucky in that scar tissue has not been a problem.
Have you been through PT yet? My guess is yes. I read where you have more pain when you lay down. I also had a big problem with this after my first 3 lumbar surgeries. Now that I have posterior rods and screws from L3-S1 I don't have the increased pain when I lay down. My NS believes prior to the rods and screws I had some posterior micromovement which meant my lumbar spine was not stable. Since you have Spondylosis that may be why you have so much more pain when you lay down.
OK I am writing way to much here so I will post more as we talk more so I don't get off track and also so you don't have to sit and read so long.
Maybe if we can work on a couple questions you have at a time instead of my trying to answer everything you wrote all at once. I want to make sure I help you the best I can,
